About the Role

Our client is seeking a proactive and empathetic Disability Support Coordinator to join their team in Benoni . This hybrid role focuses on empowering individuals with disabilities to live fulfilling and independent lives by coordinating essential support services, resources, and opportunities. You will work closely with clients, their families, and a network of service providers, blending in-person meetings and assessments with remote case management and communication.

The ideal candidate is passionate about advocacy, possesses strong organizational skills, and is adept at navigating the complexities of disability support systems to ensure clients receive personalized and effective assistance.

Key Responsibilities
  • Conduct thorough assessments of client needs and develop individualized support plans.
  • Coordinate access to services such as therapy, personal care, assistive technology, and educational programs.
  • Liaise with government agencies, healthcare providers, and community organizations to secure resources.
  • Provide ongoing support and advocacy for clients, ensuring their rights and needs are met.
  • Facilitate communication between clients, families, and service providers, using both in-person and virtual methods.
  • Maintain accurate case notes, progress reports, and administrative records.
Requirements
  • Bachelor's degree in Social Work, Psychology, Disability Studies, or a related field.
  • Minimum of 4 years of experience in disability support, case management, or a related role.
  • Understanding of disability legislation and support frameworks in South Africa .
  • Proven ability to coordinate services and manage multiple client cases.
  • Excellent interpersonal, communication, and problem-solving skills.
  • Comfortable working in a hybrid model, including remote client engagement.
