Are you looking for a new change of direction or a new challenge?, We are looking for Registered Nurses and Midwives to join our Health Visiting team as a Community Health Nurse (CHN) in Southampton City supporting children, young people and families in their own homes and community settings. We welcome applications from all Registered Nurses, Adult, Child, Mental Health, Learning Disabilities, and Registered Midwives.

You will work with children aged 0-5 and their families, alongside experienced Health Visitors to deliver the Healthy Child Programme (DH 2021).The Healthy Child Programme offers every family an evidence-based programme of interventions including screening tests, immunisations, developmental reviews and information and guidance to support parenting and healthy choices.

This role will draw on your previous knowledge and skills to ensure the best outcomes are achieved and care is delivered to children and families in Southampton at the right time and in the right place by the right professional.

  • To participate in the delivery of core contacts of the healthy child programme to children who are part of the universal and targeted caseloads and support specialist and Tier 4 families under the guise of the Health Visitor within each locality by home visits, group or education settings.
  • To undertake short- and medium-term interventions with targeted and specialist families who have been assessed by the health visitor / public health nurse as needing help with healthy weight activities, child development and/or parenting interventions. This may be by home visits or groups or interventions in an education or local authority setting.
  • Through supervision with line manager in 1 to 1s and within supervision groups the CHN will demonstrate competency in clinical decision making to assess and develop appropriate plans of care under the universal and targeted levels of the Healthy Child Programme.
  • To communicate effective, providing and receiving sensitive and complex or contentious information where advanced inter-personal skills are required.
  • Build effective relationships with other agencies and disciplines, and work in partnership to promote health and wellbeing of communities, families and individuals, and to act as an advocate where appropriate.
