This is an exciting opportunity to join the Older Person's Specialist Team (OPST) as the Team Lead, working within Division B - Medicine for Older People. The successful applicant will join a team of highly motivated, experienced, specialist nurses. The post will be based with OPST, working alongside the medical, ward & therapy teams to support patients over 80 who have complex health and social care needs.
This will be a 12 month fixed term position working 22.5 hours per week.

- Provide expert clinical knowledge and support.
- Assist the medical and nursing teams in managing caseloads of patients who have complex health and social care needs.
- Work closely with the community teams in the management of long-term conditions.
- This is an innovative opportunity for integrated working across both primary and secondary care providing a high quality, evidence based seamless service for patients.
- Proven evidence of excellent inter-personal skills and the ability to work across boundaries is essential.
