Accountable to Lead Nurse and Practice Manager

Job Purpose To provide high-quality evidence-based nursing care to the practice population, promoting health, preventing disease and supporting patients with long-term conditions. The post holder will work as part of the multidisciplinary team to deliver safe, effective and compassionate care.

• Provide treatment room nursing services.
• Deliver chronic disease management clinics, including diabetes.
• Undertake cervical cytology.
• Administer childhood and adult immunisations and vaccinations.
• Perform wound care and dressings.
• Undertake NHS Health Checks.
• Monitor and recall patients with long-term conditions.
• Support patients with self-management and lifestyle advice.
• Maintain accurate clinical records using EMIS.
• Participate in infection prevention and control activities.
• Maintain professional registration and undertake mandatory training.
• Participate in clinical audit, quality improvement and practice meetings.

• Registered Nurse (NMC).
• Evidence of continuing professional development.
• Experience Experience delivering nursing care.
• Excellent communication and interpersonal skills.
• Ability to work independently and within a multidisciplinary team.

• General Practice Nursing experience.
• Experience managing long-term conditions.
• Diabetes qualification or experience.
