The Role:As an Assistant Family Practitioner
, you will play a key role in helping children, young people and families achieve positive, lasting change. Working with families of children aged 0–19, you will provide early help and targeted support that builds on family strengths, promotes resilience and helps children thrive safely at home.
Working as part of a supportive and dedicated team, you will deliver one‑to‑one support, outreach appointments and evidence‑based programmes, helping families identify goals, overcome challenges and access the right support at the right time. Through strong partnership working and a solution‑focused approach, you will contribute to improving outcomes for families across your local community.
What you’ll do:- Deliver evidence‑based parenting programmes, targeted support groups and interventions that help children, young people and families achieve positive outcomes.
- Provide one‑to‑one, outreach and session‑based support, helping families build on their strengths, overcome challenges and achieve agreed goals.
- Offer advice and guidance, signpost families to appropriate services, and identify and elevate safeguarding concerns in line with procedures.
- Develop and maintain knowledge of local services, community resources and support networks, helping families access the right support at the right time.
- Maintain accurate records, gather feedback and contribute to continuous improvement and high‑quality service delivery.
- Level 2 qualification (or above) in early years, education, health, child development or a related field.
- Experience (paid or voluntary) of working with children and young people aged 0–19 and their families.
- Experience of multi‑agency working to support children and families with complex needs.
- Excellent communication, relationship‑building, mediation and negotiation skills.
- A commitment to inclusive practice, professional integrity, continuous learning and delivering positive outcomes for families.
