Waythrough is recruiting a Complex Needs Practitioner to deliver intensive, person‑centred support across Shropshire and Telford & Wrekin. You will work in a multidisciplinary team, managing your own caseload while coordinating support with partner agencies to ensure timely help.
You’ll develop personalised recovery plans, deliver evidence‑based interventions and help individuals access services to improve health, wellbeing and independence. This is outreach‑based and impact‑driven work.
