Continuing Healthcare (CHC) Independence Advisor About the Role

We are seeking a knowledgeable and experienced CHC Independence Advisor to support our Adult Social Care CHC team. This non-registered role is ideal for someone with a strong understanding of the NHS Continuing Healthcare (CHC) Framework and the Mental Capacity Act, with hands‑on experience completing CHC Checklists.

Please note this post will require travel to carry out lone working visits to individuals in their own homes, predominantly in the Southampton area, and will require successful candidate to work a minimum of one day in the office at the civic each week, subject to change in accordance with business needs.

Key Responsibilities
  • Support Adult Social Care teams with CHC referrals by completing CHC Checklists and ensuring appropriate consent processes are followed, including MCA and Best Interest decisions. This will require face to face visits to people in their own homes, and ability to lone work.
  • Explain the CHC process clearly to individuals and their representatives to support informed decision‑making.
  • Advise care providers on documenting supporting information.
  • Work collaboratively with health and social care professionals
Essential Criteria

About the team

You will join an energetic and supportive team striving to support people in Southampton live active, safe and independent lives. The Adult Social Care CHC Team represents Southampton City Council Adult Social Care at Decision Support Tool (DST) Multi‑Disciplinary Team (MDT) meetings to determine NHS Continuing Healthcare (CHC) eligibility. The team works collaboratively with health partners to agree joint funding arrangements, including Section 117 aftercare (in liaison with relevant adult social care teams), and provides expert advice to social care teams on complex health and care needs.

Their role is central to ensuring integrated, person‑centred support and fostering strong inter‑agency relationships.

Salary: The salary band for this role is £37,280 - £40,778 per annum. The starting salary is £37,280 with annual progression through the salary bands.
