All screens Nationwide Ltd. is seeking an experienced Hospice Social Worker to support patients and families across inpatient and community settings. You will assess social circumstances, advocate for needs, and lead complex discharge planning as part of the MDT.
You will guide care planning, ensure rights under the Mental Capacity Act 2005, and stay aligned with Care Act 2014 requirements. Regular CPD and collaboration with care funding bodies are essential.
