Primary Care/Geriatrics Physician – Southbury, CT

Primary Care/Geriatrics Physician opening in Southbury, Connecticut (85 miles from New York City). Nuvance Health Medical Group is seeking a physician to provide comprehensive outpatient care for adult patients in a family‑friendly, practice‑based setting.

• Eleven providers – 8 physicians & 3 nurse practitioners.

• Established, highly regarded practice.

• Minimum on‑call obligations.

•

Location:

Southbury, CT – 80 miles from NYC, 40 miles from Hartford.

• On‑site SNF coverage and teaching opportunities for medical students/residents.

• Full‑time, office‑based outpatient primary care in a team‑based environment.

• Manage acute and chronic conditions for adult patients.

• Collaborate with multidisciplinary support staff to promote health and well‑being of panel of covered lives.

• Utilize state‑of‑the‑art Electronic Health Record fully integrated across all hospitals and sites.

• Provide after‑hour coverage – light call volume shared among providers with phone‑calls only (after nurse triages first line).

• MD or DO with CT and NY state license, Federal DEA in both states and CT State Controlled Substance Registration.
• Successfully completed an Internal Medicine residency. Geriatrics fellowship a plus.
• Board Certified or Board Eligible.
• Experience with after‑hour light call coverage.

Competitive total compensation package including base salary, incentive pay, paid time off, CME time off and allowance, medical, dental, vision, life, disability, and 401(k) match. Sign‑on bonus and relocation allowance (if relevant) are offered. Salary range: $275,000 – $285,000 annually commensurate with experience.
