RVNAhealth is seeking a compassionate Live-In Nurse (LPN preferred; RN considered) to provide one-on-one care in a private home setting. This per diem role supports a patient transitioning from rehab to home.
Location: Southbury, CT
Start: 2nd week of January (pending patient discharge from rehab)
Duration: 1-2 months
Schedule: Live-in; 12.5 hours/day (breaks included)
- Provide direct patient care in the home
- Complete patient assessments and develop/update plans of care
- Coordinate care and communicate with physicians
- Monitor patient status and respond to changes in condition
- Maintain accurate, timely documentation per RVNA policies
- Educate patients and families on wellness and safety
- Follow all agency policies, HIPAA, infection control, and regulatory standards
- Participate in required in‑services and maintain CPR certification
- Graduate of an accredited nursing program
- Current CT RN or LPN license (LPN preferred) or Compact license required (active and in good standing)
- Minimum 1 year of nursing experience (hospital or home health preferred)
- Strong documentation and communication skills
- Computer literate
