Southend Integrated Healthcare for the Homeless (SIHH) is seeking an enthusiastic and compassionate Prescribing Paramedic or Nurse to join our dedicated team, providing specialist healthcare services to people experiencing homelessness across Southend-on-Sea.
This is an opportunity to work beyond the traditional clinical setting, delivering meaningful, person‑centred care and helping individuals overcome barriers to accessing healthcare. Every day offers the chance to make a genuine impact on health outcomes and improve lives.
What you’ll be doing- Deliver specialist primary care consultations and outreach clinics within community settings, including local soup kitchens and homeless support organisations.
- Conduct comprehensive health assessments, including general physical health reviews, health screening and prevention, wound assessment and management, lifestyle and wellbeing assessments, and promote health and wellbeing through proactive interventions and health education.
- Improve access to healthcare by supporting engagement with primary care services, secondary care providers, mental health services, drug and alcohol services, and community and voluntary sector organisations.
- Develop and maintain effective pathways into specialist services to ensure patients receive the right care at the right time.
- Act as a key point of contact between healthcare providers and community services.
- Deliver regular drop‑in clinics across various homeless support locations in Southend.
- Champion preventative healthcare, including promoting female health screening and wider public health initiatives.
- A qualified Prescribing Paramedic or Registered Nurse Prescriber.
- Passionate about tackling health inequalities.
- Comfortable working autonomously within a community setting.
- Compassionate, resilient and non‑judgemental.
- An excellent communicator with strong relationship‑building skills.
- Committed to delivering high‑quality, person‑centred care.
