Verse One Group is seeking a bright nurse to join the Integrated Discharge team at Southend Hospital on a fixed-term basis to cover maternity leave from 22/02/2027 to 21/11/2027. The post focuses on assessing patients ready for discharge and ensuring appropriate community referrals.
The role requires excellent communication with patients, relatives, the MDT and system partners to facilitate safe discharges and maintain patient flow within the acute trust.
