Intake Representative – PACE Southeast Michigan

Under the supervision of the Intake Manager, the Intake Representative coordinates the PACE intake process, performs initial screening, informs potential participants of eligibility requirements, maintains the Home Visit schedule, and reports intake activity data collection. The role serves as the initial liaison between potential participants, their families, and the PACE team, delivering information about the PACE program and other community services through phone contact to promote enrollment.

Duties and Responsibilities
  • Respond to inquiry calls, provide information, conduct initial phone screening, and assist Outreach Liaisons with scheduling home visits for referrals.
  • Document intake activity through completion of phone screens, documentation in Tru Chart referral module, and communication with the Intake team.
  • Complete quarterly referral and activity reports; input participant information into various databases.
  • Maintain the Enrollment module in Tru Chart.
  • Communicate with Home Visit staff for daily home visit assignments.
  • Coordinate timely follow‑up with referral sources, families, and participants via phone, letters, and other appropriate documentation.
  • Assist in community outreach activities as assigned and represent PACE at community events.
  • Maintain confidentiality of all PACE or prospective PACE participant information and other sensitive information.
  • Perform other tasks as assigned.
Knowledge, Skills, and Abilities
  • High School diploma or GED required.
  • At least one year of experience working with the elderly in a community‑based setting.
  • Knowledge of senior community and local resources.
  • Ability to work effectively with diverse populations, including elderly.
  • Capability to function in a multi‑task environment.
