SPEECH THERAPIST

Under the general direction of the Rehabilitation Services Manager, with input from the Clinical Nutrition Manager, the Speech Language Pathologist (SLP) is responsible for assessing, diagnosing, and treating speech, language, social communication, cognitive‑communication, and swallowing disorders for participants enrolled in PACE Southeast Michigan. The SLP determines the appropriate plan of care based on clinical knowledge, implements individualized treatment plans, monitors effectiveness, and collaborates with all members of the interdisciplinary team (IDT), most directly with the clinical registered dietitians and the rehab team.

Specific Duties and Functions
  • Completes in‑person initial comprehensive speech therapy assessment/re‑assessment as requested by the IDT team.
  • Participates in IDT and other meetings as needed; effectively communicates significant changes and collaborates with the IDT to determine the participants plan of care as needed.
  • Completes documentation that is relevant, accurate, and timely in the participant’s electronic medical record in accordance with department standards.
  • Provides education to the participant and/or caregiver regarding the treatment plan and recommendations.
  • Collaborates and communicates the treatment plan with all necessary PACE team members, including the clinical registered dietitians, rehab team, dietary aides, and participant care associates, to ensure compliance with recommendations, participant safety and other needs for safe 24‑hour care.
  • Acts as a liaison between PACE Southeast Michigan and external healthcare providers regarding speech therapy needs and recommendations.
  • Acts as the primary internal consultant for communication devices.
  • Assists with data collection, prepares reports, and maintains any necessary participant statistics. Conducts and participates in clinic educational programs.
  • Acts as primary consultant for clinical evaluation of swallowing and diet issues. Works with dietary and day center staff on participant swallowing issues and appropriate diet textures.
  • Understands, complies with, and promotes the Participant Bill of Rights and works to achieve high levels of participant satisfaction.
  • Performs other duties as required or requested.
Knowledge, Skills, and Abilities Required
  • Must have a master’s degree in Speech and Language Pathology with a Clinical Certification of Competency in Speech Pathology (CCC).
  • Be legally authorized, currently licensed, registered or certified if applicable to practice in the state of employment.
  • Must have one year of experience with a frail, elderly population.
Work Environment
  • Participant care environments at multiple sites with potential exposure to communicable diseases.
  • Limited noise, temperature fluctuation and dust are possible.
