Join us at Rivetus Rehab! As an OT in this role, you will become an integral teammate in our innovative Take Your Therapist Home Program.

What is Take Your Therapist Home? In a world of shorter SNF stays, Take Your Therapist Home (TYTH) is a one-of-a-kind program where you as the clinician work directly with patients in the SNF and home health, providing a continuity of care to have a greater impact on patient outcomes. You will be primarily in the home health setting with dedicated time and caseload support for the SNF.

You will have a localized coverage area surrounding the SNF. Your schedule is based on a traditional work week.

Rivetus has been serving the geriatric post-acute community since 1998. We are a specialized rehabilitation company operated and managed at the executive level by licensed clinicians, providing continuity of care across Skilled Nursing, Home Health, and Outpatient settings.

• Provide OT services in the TYTH program in home health and SNF settings with continuity of care.
• Collaborate with patients, families, and care teams to optimize outcomes.
• Manage a localized service area with a traditional work week schedule.

• Successful completion of an accredited Occupational Therapy program.
• Licensure as required by the State of Michigan.

• Ability to exercise professional judgement and initiative when analyzing problems and recommending solutions.
• Initiative and flexibility in the work environment.
• Ability to respond effectively to the needs of patients and their families.
• Physical ability to bend, position, transfer and lift patients weight from 80 pounds to over 300 pounds, using lifting devices clinically recommended for the patient.
