As a PTA in this role, you will become an integral teammate in our innovative Take Your Therapist Home Program. This role is focused on Home Health Care in the greater Southfield area with a steady referral source from multiple skilled nursing facilities.
What is Take Your Therapist Home?In a world of shorter and shorter SNF stays, Take Your Therapist Home (TYTH) is a unique program where clinicians work directly with patients in the SNF and home health providing a continuity of care allowing for you to have a greater impact on patient outcomes! Rivetus has been serving the geriatric Post‑Acute community since 1998. We are a specialized rehabilitation company operated and managed at the executive level by licensed clinicians.

Evergreen Health and Rehab Center – 19933 W. Thirteen Mile Southfield, MI 48076. This is a hybrid role providing Home Health therapy services in conjunction with SNF based therapy services. Home Health geographic area is the vicinity around our Evergreen/Southfield facility. Home care services are paid per visit, and SNF services are paid at an hourly rate. Ideal candidates would be on the west side of Commerce/Wixom area.
ScheduleMonday – Friday, Full Time.
Required Qualifications- Successful completion of an accredited Physical Therapy Assistant program.
- Licensure as required by the State of Michigan.
- Ability to exercise professional judgement and initiative when analyzing problems and recommending solutions.
- Initiative and flexibility in the work environment.
- Ability to react effectively to the needs of patients and their families.
- Physical ability to bend, position, transfer and lift patients weighing from 80 pounds to over 300 pounds, without restriction, utilizing lifting devices clinically recommended for the patient.
