WhatYou'll Do
- Conduct in‑depth mental health assessments to identify patient needs.
- Develop, implement, and evaluate individualized treatment plans.
- Provide outpatient therapy tailored to adults and/or children with diverse mental health needs.
- Offer crisis intervention services, ensuring immediate support and patient safety.
- Work closely with interdisciplinary teams to provide holistic patient care.
- Establish and maintain relationships with community organizations for patient advocacy.
- Connect individuals with mental health and public health resources to enhance their support network.
- Maintain accurate patient records, treatment plans, and progress reports.
- Conduct regular assessments to evaluate patient improvement and adjust treatment strategies.
- Prepare detailed case summaries and discharge plans.
Required Qualifications
Education:
Masters Degree in Social Work (MSW).
License:
Active LMSW licensure.
Experience:
At least 3 years of experience working with diverse populations, including expertise in:
- Conducting mental health assessments and developing treatment plans.
- Providing individual and family therapy.
- Crisis intervention and behavioral health support.
- Proficiency in evidence‑based practices and psychosocial assessment.
- Strong treatment planning skills with the ability to adapt plans as needed.
- Excellent communication skills for patient counseling and interdisciplinary collaboration.
- Ability to handle crisis stabilization and make emergency decisions.
- Familiarity with local mental health resources for patient referrals and advocacy.
