• Assess patient health, develop and implement nursing care plans, and monitor recovery.
• Direct Patient Care: Administer medications, provide wound care, monitor vital signs, and assist with therapies and medical equipment.

  Teach patients and caregivers about conditions, medications, and home management.
• Collaboration and Communication: Work with physicians and other professionals to coordinate care and report on progress.
• Documentation: Maintain accurate and detailed medical records of assessments, care, and education.
• Supervision and Training: Supervise and train other personnel such as home health aides.

  Skills:
  
  Proficient in clinical assessment and use of equipment (blood pressure monitors, glucose meters, feeding pumps).

• Experience with Electronic Health Record (EHR) systems.
• Exceptional communication, time management, organizational, and problem‑solving skills; compassion and ability to work independently.

• Registered Nurse (RN) license required;
  Bachelor of Science in Nursing (BSN) preferred.
• Valid driver’s license and reliable transportation for travel between patient homes.
