Overview
Licensed Practical Nurse (LPN) role at HOPE At Home Health Care
We are currently seeking a Licensed Practical Nurse (LPN) to join our Team. Essential Responsibilities and Procedures- Provide skilled professional nursing care within the scope of LVN practice, to the patient as prescribed by the physician; comply with regulations established by the State Board of Nursing, American Nursing Association, and agency policies and procedures. Skills are demonstrated by skills visits and by providing services in accordance with agency policies.
- Prepare clinical and progress notes.
- Assist the Registered Nurse or physician in performing specialized procedures and duties.
- Assist the Registered Nurse in carrying out the plan of care.
- Assist the client in learning appropriate self-care techniques.
- Assist patient and family/caregiver in maintaining a safe home environment.
- Involve patient/family/caregiver in the care process to support self-sufficiency and independence.
- Observe the patient for changes in condition and report changes to the case manager and other supervisory personnel.
- Prioritize appropriate teachings/interventions as indicated by the clinical picture.
- Utilize appropriate resources to function effectively in the home care setting.
- Participate in established Agency committees as requested by supervisor.
- Attend and participate in mandatory Agency staff meetings, case conferences, educational offerings, and training programs.
- Maintain and upgrade professional knowledge and practice skills through continuing education and submission of required documentation according to State requirements.
- Use the Case Manager as the first resource for problems or questions; contact the Supervisor if problems arise in reaching the Case Manager.
- Participate in quarterly utilization reviews and chart audits as required by the agency.
- Demonstrate timely response to Agency Communication System.
- Maintain Agency property, supplies, and equipment in a responsible and accountable manner.
- Demonstrate infection control per OSHA guidelines and Agency policies.
- Ensure compliance with Medicare Conditions of Participation and other regulatory requirements.
- Possess and maintain a current CPR certificate and Tuberculin testing.
- Continuously analyze work processes and suggest improvements.
- Graduate of an accredited School of Nursing and current licensure in the state of practice.
- One year of recent experience in adult patient home care (required).
- Excellent observational and assessment skills and nursing judgment; good verbal and written communication skills.
- Must be a licensed driver with an insured automobile in good working order.
