Position Overview

We are seeking an experienced Registered Nurse (RN) / Case Manager to join a supportive hospice care team providing community-based care to patients in their homes.

The RN/Case Manager will coordinate and provide hospice services based on each patient's individualized plan of care. This position involves comprehensive patient assessments, care planning, family education, clinical documentation, and collaboration with physicians and the interdisciplinary team.

Key Responsibilities
  • Provide and coordinate hospice care according to individualized patient care plans.
  • Conduct initial and ongoing comprehensive patient assessments.
  • Review medical histories and evaluate patients' physical and emotional conditions.
  • Monitor vital signs, mental status, appetite, dietary needs, and changes in condition.
  • Assess patients' home environments and available family support.
  • Develop, implement, and update individualized plans of care.
  • Coordinate services with physicians, hospice staff, contracted providers, and the interdisciplinary group.
  • Communicate significant changes in patient condition to physicians and appropriate care team members.
  • Educate patients and families regarding the plan of care, medications, end-of-life processes, and healthcare decisions.
Qualifications
  • Graduate of an accredited School of Nursing.
  • Active RN license in the applicable state.
  • 1-2 years of hospice experience required.
  • Strong patient assessment and clinical problem-solving skills.
  • Knowledge of hospice and case management processes.
  • Valid driver's license.
