About the role

Hartford Health Care at Home provides homecare services. As a Social Worker (MSW) you will engage patients in their homes to determine needs and provide counseling to meet care plan objectives. You will collaborate with patients, families, physicians, and the home care team.

Key responsibilities
  • Collaborate with the interdisciplinary team to identify patients who would benefit from Social Work Services.
  • Complete comprehensive in‑home psychosocial evaluations, considering developmental, social, cultural, substance abuse, psychological, environmental, and medical issues.
  • Assess physical, social, emotional, and familial characteristics using evidence‑based assessment tools (e.g., PHQ‑9/PHQ‑2, SLUMS, BIMS, FAST, SAD Caregiver Strain Index) to determine cognitive status and behavioral health needs.
  • Identify home safety, family dynamics, caregiver strain, other risk factors including suicide, and patient/family understanding of disease treatment options.
  • Determine appropriate level of care, including needs for skilled‑nursing facilities, assisted living, adult day care, etc., and develop a culturally competent care plan with measurable goals.
  • Implement short‑term interventions, such as triage, crisis intervention, individual or family therapy, screening, and planning to address barriers and promote health and well‑being.
  • Utilize Motivational Interviewing and Coaching skills to assist patients/families in setting and achieving goals, encouraging self‑determination and independence.
  • Act as an educator for patients, families, community, and other professionals regarding disease prevention, progression, impact, health maintenance, and treatment adherence.
  • Advocate for patients’ needs to improve access to care and delivery of services.
  • Provide ongoing assessment of patient and family needs; monitor and adjust care plan goals and interventions as required.
  • Maintain timely documentation of social work services reflecting assessment and treatment outcomes.
  • Facilitate team‑based care and collaboration, working with primary care, specialty physicians, and others to improve quality of life.
  • Participate in case conferences and interdisciplinary team meetings; act as consultant for agency staff regarding abuse, neglect, exploitation, and community resources.
  • Identify and link patients and families to Home Health Care (HHC) and community‑based resources as appropriate.
Qualifications
  • 2–3 years of healthcare experience in acute and community settings.
  • Licensed MSW (LMSW required; LCSW preferred).
  • Knowledge of social, cultural, economic, political, religious, medical, psychological, and legal issues influencing patient and family behavior.
  • Familiarity with evidence‑based assessment tools (PHQ‑9/PHQ‑2, SLUMS, BIMS, FAST, SAD Caregiver Strain Index, etc.).
  • Ability to obtain community resources (town, state, federal programs and eligibility requirements).
