The Social Worker provides services to patients, their caregivers, and families that are needed to assist the patient and/or family with solving personal, social, and financial problems that may interfere with the patient’s, caregiver’s, and family’s quality of life. Social‑work interventions are provided under the direction of a physician and in accordance with the plan of care.
Responsibilities- Provides culturally sensitive, non‑judgmental end‑of‑life care before and after death.
- Conducts comprehensive assessments and evaluations of patient, caregiver, and family psychosocial needs and connects them with community resources as needed.
- Assesses needs, strengths, and coping skills; identifies support systems and community resources to reduce stress and facilitate coping with end‑of‑life care.
- Makes appropriate community referrals in accordance with patient and family preferences.
- Assesses patient’s appropriateness for Live‑Alone Program and regularly evaluates patient and caregiver ability to provide safe care.
- Assesses environmental and financial resources and makes referrals as needed.
- Provides social‑work interventions and ensures psychosocial assessments address issues impacted by terminal diagnosis and symptoms.
- Conducts ongoing assessment during any contact or interaction related to the patient, caregiver, and family.
- Provides short‑term individual and family counseling for specific problems or symptom relief.
- Provides advocacy and ensures interventions are initiated according to patient and family preferences.
- Enhances the strengths of the family system and refers for ongoing bereavement services.
- Maintains dignity of the dying patient and the grieving family, supporting unique spiritual and cultural beliefs.
- Provides holistic family‑centered care across treatment settings and appropriate crisis intervention as needed.
- Assists with decision‑making and care planning, prepares advance directives, and provides information for funeral planning according to preferences.
- Assists IDG in understanding emotional factors and helps team members address issues in accordance with patient preferences.
- Reports abuse and neglect to the primary team and appropriate community protective agencies.
- Documents evaluations, assessments, and interventions; writes clinical notes after every contact with basic content, progress, and future plans.
- Completes plans of social work treatment including goals, intervention plans, and implementation; updates MSW physician orders; participates in Plan of Care development and revision.
- Maintains professional knowledge through education, workshops, and in‑services; participates in continuing education required by state and federal regulations; provides in‑service training as needed.
- Ensures strategic community visibility and development goals are met; responds to community requests for information and education; participates in public speaking engagements; serves as liaison to community extended‑care facilities and inpatient providers.
- Master’s degree in social work from a school accredited by the Council on Social Work Education with a minimum of three (3) years clinical experience in Social Work preferred.
