This role coordinates communication between the contracted monitoring company and clinical staff. It assists the Telemonitor RN in the daily management and delivery of care of patients currently on Telemonitoring service and collaborates with the Telemonitor RN and clinical team members to achieve exceptional outcomes. The role also consults with the Telemonitor RN and team to provide in‑home Telemonitor installs for patients.

Responsibilities
  • Identify high‑risk patients requiring conference with case manager based on high alert frequency related to parameter settings or patient reporting of problems.
  • Investigate reason for high frequency of empty packet alerts for patients.
  • Check on suspension of TM monitoring if reported/noted greater than 3 days.
  • Conferences with case manager/MD office regarding TM services.
  • Participate in investigations/reviews of adverse outcomes for TM patients.
  • Collaborate with Tele‑monitor assistants on installations, troubleshooting and discharge of patients.
  • Review daily task assignments for installations, TS, and discharge of patients.
  • Review patient’s medical record for orders/parameters for TM services.
  • Collect weekly deployment data, re‑hospitalization of TM patients reports, and other metrics as assigned to quality.
  • Document in electronic medical record, calls, case conferences, and clinical notes related to patient’s care and TM service.
  • Coordinate email/voicemail alerts from contracted monitoring company.
  • Distribute clinical and non‑clinical information from monitoring company to appropriate clinical teams, managers, and staff.
  • Actively participate in the improvement process known as H3W to identify TM service line needs and development.
  • Perform other duties as assigned.
Qualifications

Education: High school diploma or GED required. Successful completion of a CT state‑approved Licensed Practical Nurse certificate program required. Licensed RN preferred.

Experience: 2‑3 years as a Licensed Practical Nurse required. Experience in care coordination, case management, ambulatory care, telehealth, or critical care highly preferred. Proficiency with Electronic Health Records (EHR/EMR) and remote monitoring platforms. 3‑5 years of RN experience, particularly in critical care, ambulatory care, case management, or chronic care preferred. Experience with virtual platforms, remote monitoring tools, and delivering care via phone/video preferred.

Managing patients with comorbidities (CHF, COPD, Diabetes) preferred.

Licensure, Certification, Registration: Current and valid LPN required. Valid CT driver’s license with a good driving history required.

Knowledge, Skills and Ability Requirements: Computer skills including Outlook, Microsoft Word, and Excel. Excellent organizational and time‑management skills. Ability to provide excellent customer service.
