Our Cardiovascular Data Management Center provides comprehensive and continuous quality improvement information and allows us to compare patient outcomes to state and national benchmarks.

• The APP will divide their time between SNF setting and infusion/outpatient cardiology clinic.
• The infusion clinic involves assessment, follow‑up, instruction, and collaboration with the community cardiologist for the necessity of intravenous diuresis, thereby reducing the need for hospitalization, improving QoL, and implementing GDMT.
• The APP will be responsible for instituting a new infusion clinic needed across the central region outside of the current one established at HOCC, and for coverage of cardiac testing.
• The APP will play a vital role in filling the gaps and needs of our heart‑failure population, educating patients and families on lifestyle modifications, GDMT, and providing an additional resource to affiliated cardiology practices.

Full Time – Hours split TBD by the needs of patient population. The APP is primarily in SNF setting.

• Direct support from the Chief of Cardiology to help create a new cardiology consultation program within the Skilled Nursing Facility setting.
• Autonomy to create your own workflow and schedule.
• Only travel between sites located closely together.

• Graduate of an accredited Physician Assistant or Nurse Practitioner program.
• Licensed, or eligible to be licensed, as a Physician Assistant, or Advanced Practice Registered Nurse (APRN) in the state of Connecticut.
• Certification or eligible for certification as a Physician Assistant or National Board Certification as an Advanced Practice Registered Nurse.
• Cardiology experience required.
• Geriatric board certification preferred.
