Home Care LPN — Impactful Patient-Centered Nursing

Hartford Health Care at Home is seeking a licensed practical nurse to provide intermittent skilled nursing care to clients in home and community settings under the supervision of an RN. You will follow the plan of care and support patient outcomes in the home.

CT LPN licensure, valid CT driver's license, and EMR proficiency are required. You will record daily progress, communicate with the primary nurse and other disciplines, and participate in care planning with a multidisciplinary team.
