Community Facilitator - Social Support

Our client is seeking a compassionate and dedicated Community Facilitator to join their Social Support team based in Soweto . This role is central to connecting community members with essential services, resources, and support networks. You will act as a key point of contact, identifying needs, providing guidance, and empowering individuals and families to improve their well-being and overcome challenges. • Engage with community members to identify their social support needs and challenges.
• Provide information and referrals to relevant services, programs, and resources.
• Facilitate access to essential support systems such as healthcare, education, and social grants.

• Build and maintain positive working relationships with local organizations and service providers.
• Maintain accurate records of client interactions, referrals, and follow-ups.

• Minimum of a Grade 12 certificate; relevant tertiary qualification is advantageous.
• Experience in a community-based role, social support, or client services.
• Good understanding of social services and resources available in the Soweto area.
• Strong interpersonal, communication, and listening skills.
• Ability to work empathetically with diverse populations.
• Willingness to travel within the community and work flexible hours as needed.
