Wellness Home Services Licensed Practical Nurse (LPN)

We are seeking a Licensed Practical Nurse to provide individualized, one‑on‑one care for residents in our wellness home services program at Acts Retirement‑Life Communities.

Responsibilities

• Deliver patient‑centered care tailored to each resident’s unique needs under the direction of a Registered Nurse, where required by state guidelines.
• Specialize in managing chronic conditions, supporting post‑operative recovery, and addressing complex medical needs with safety, dignity, and comfort.
• Perform medication management, vital sign monitoring, and collaborate with multidisciplinary care teams.
• Provide clear education and guidance to residents and families to promote understanding and health outcomes.

Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing with the applicable state Board of Nursing.
• Minimum of 1‑2 years of clinical nursing experience, preferably in acute care or home health.
• Current CPR certification (or obtain within three months of employment).
• If responsible for transporting a resident, must maintain a valid state driver’s license and provide proof of current automobile liability insurance.

Pay Range $23.69 – $28.75 per hour, starting rate based on skills and experience.
