American Traveler is seeking a Home Health Physical Therapist with 2+ years of home health experience and HCHB/Oasis charting proficiency.
Requirements- Minimum 2 years of home health experience required
- Active state PT license required (license in hand, no pending licenses accepted)
- License required if applicable
- Current BLS certification required
- HCHB and Oasis charting experience required
- Home health setting covering geriatric patients with cases including start of care and follow-up visits
- Caseload of 6-7 patients per day, potentially 3 visits worth of points
- Minimum productivity requirement of 30 points per week
- Day shift schedule, 5x8 hours, 8:00 AM to 5:00 PM
- No weekend requirement unless needed to meet weekly productivity targets
- No on-call requirements
- Territory spans White, Cumberland, and Van Buren Counties with a 50-75 mile coverage radius
- Mileage reimbursed at IRS standard rate
- Uses HCHB and Oasis for charting
- Dress code is scrubs or business casual; PTs wear a polo with scrub pants
- First-time travelers will not be considered for this position
