Responsibilities
Provides individualized medical nutrition therapy to ambulatory and hospitalized patients as a part of a multidisciplinary team in a tertiary care center. Serves as a consultant on nutrition care for other members of the health care team. Applies evidenced-based medical nutrition therapy standards of care to incorporate the nutrition care process: nutrition assessment/re-assessment, nutrition diagnosis, nutrition intervention, and nutrition monitoring/evaluation.
Completes thorough assessment, including nutrition focused physical exams, review of medical record for anthropometrics, medical history, labs, food allergies/intolerances, and drug-nutrient interactions. Identifies diagnostic criteria, including malnutrition. Interventions include developing advanced/specialized nutrition plans and writing orders/recommendations for therapeutic diets, oral nutrition supplementation, vitamin and mineral repletion, and nutrition support (enteral and parenteral) for high acuity/medically complex patients.
Provides nutrition education/ counseling utilizing principles of motivational interviewing. Empowers family and caregivers to support medically complex patients with therapeutic diet prescribed. Coordinates ongoing nutrition care for patients, facilities, caregivers, and durable medical equipment companies.
Qualifications
Bachelors degree in Nutrition or related field. Graduate of an Accreditation Council for Education in Nutrition and Dietetics (ACEND) accredited internship or approved training program. Licensed or eligible for licensure in the state of Wisconsin as a Licensed Dietitian.
