About UsCenter Well Senior Primary Care provides proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, minor injury treatment and more. Our integrated care teams—composed of physicians, nurses, behavioral health specialists and more—spend up to 50 percent more time with patients to provide compassionate, personalized care that brings better health outcomes.

Base pay range: $ / yr – $ / yr.
Full‑time hours (40 hrs per week) with salary adjusted for skill, experience and location.
Your Role & Responsibilities- Provide comprehensive, evidence‑based care to seniors in accordance with clinical standards.
- Collaborate daily with the interdisciplinary care team to coordinate high‑quality outcomes.
- Manage referrals, hospital/SNF coordination, durable medical equipment (DME) and home‑health services.
- Participate in daily huddles and on‑call rotations supporting center growth initiatives.
- Document care accurately and efficiently, supported by quality‑based coding staff.
- Ensure compliance with all licensing, accreditation and regulatory requirements.
- Master’s degree in Nursing or completion of a PA program with board certification.
- Active, unrestricted NP or PA license in the state of practice.
- Strong communication and interpersonal skills.
- Passion for value‑based care and integrated team‑based models.
- Basic computer proficiency (EMR, email, etc.).
- Experience in value‑based primary care or Medicare Advantage settings.
- Active DEA license and Medicare/Medicaid provider numbers or ability to obtain them.
- Familiarity with HEDIS quality indicators and Medicare guidelines.
