Job PurposeThe Clinical Pharmacist will help advance quality pharmacotherapy care by actively assuring the provision of safe, efficacious and cost‑effective drug therapy for all oncology patients. The pharmacist works in an outpatient oncology practice and collaborates with all team members utilizing a collaborative practice‑based care team model. They provide face‑to‑face and phone‑based disease management, review prior authorization requests, and communicate with health‑care professionals and patients about medication therapy and quality of life.
The pharmacist consults with patients to identify, resolve and prevent medication‑related problems and assesses each medication for indication, effectiveness, safety and adherence.
- Medication therapy management: review and monitor medication regimens, advise clinicians on evidence‑based therapy, document interventions.
- Review patient clinical chart information to determine appropriateness of requested treatment.
- Monitor and review prior authorization requests for medications.
- Develop clinical criteria and medical policies for medication review.
- Ensure all clinical criteria comply with CMS regulations and FDA guidance, clinical guidelines and evidence‑based literature.
- Manage, organize, and update relevant data using database applications.
- Provide observations and recommendations regarding operational issues, productivity, quality and customer‑service standards.
- Identify trends, resolve problems, recommend improvements, and implement change.
- Actively participate in process improvement initiatives.
- Protect organization’s value by maintaining confidentiality of information.
- Continuously update job knowledge through educational opportunities, professional publications and organizations.

- Required
Licensure/Certification :
Pharm
D and licensed pharmacist in the state of employment. - Years of
Experience:
Minimum 1 year as a clinical pharmacist with completion of PGY1 residency; PGY2 residency specializing in oncology preferred. - Skills:
Strong analytical and judgmental skills; excellent verbal communication; deep knowledge of pharmacology developments and FDA regulations; process‑management; technical and clinical proficiency; computer proficiency (knowledge of Pioneer Rx a plus). Knowledge of specialty pharmaceuticals and billing practices. - Traits:
Organized, high‑integrity, detail‑oriented, dependable, quality‑focused, empathetic, a good listener and communicator. - Other:
Energetic, highly motivated team player with discretion and confidentiality.
