Nurse Practitioner - Wound Care Rockhill/Spartanburg
We are seeking a qualified Nurse Practitioner that specializes in Wound Care to service our patients throughout the Rock Hill/Spartanburg area. As a provider, you will play a critical role in delivering exceptional patient care across various settings, including homes, facilities and telehealth. Your responsibilities include evaluating high-risk patients, coordinating care plans, and providing care management to improve patient outcomes.
Our providers conduct virtual patient visits from the comfort of their own homes as well as in-person visits in various settings including homes and facilities. This is a full-time salary-based position with the opportunity to receive a monthly bonus.
Available Shift(s)- 8‑hour shift (Monday‑Friday; 8:00am‑5:00pm)
- 10‑hour shift (Monday‑Thursday; 7:00am‑6:00pm)
- 10‑hour shift (Tuesday‑Friday; 7:00am‑6:00pm)

A successful provider will:
- Evaluate high‑risk patients with complex medical needs and develop individualized care plans.
- Coordinate care activities with other interdisciplinary team members to ensure continuity of care and optimal outcomes.
- Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members.
- Participate in interdisciplinary team meetings to discuss patient progress, treatment plans, and care coordination efforts.
- Provide patients with education, resources, and self‑management strategies to promote preventive care.
- Document care management activities accurately and efficiently to support patient care plans and regulatory compliance.
- Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.
- Monitor quality measures and identify opportunities for improvement, implementing corrective actions as needed.
- Valid Nurse Practitioner (NP), Physician Assistant (PA), or Physician license in the state of employment; license must be in good standing.
- Demonstrated experience in patient care, preferably in home health, clinic, facility, telehealth, or specialty care settings.
- Strong communication, collaboration, and interpersonal skills; ability to work effectively within an interdisciplinary team.
- Ability to evaluate high‑risk patients, develop care plans, and coordinate care activities to meet patient needs.
- Commitment to delivering compassionate, patient‑centered care and promoting a culture of excellence.
- Strong organizational and time‑management skills; ability to multitask and prioritize responsibilities effectively.
- Basic computer knowledge and effective written and verbal communication skills.
- Ability to manage travel between offices and facilities; licensed driver with a valid, insured automobile.
