Job Title: Nurse Practitioner (Mobile Healthcare Provider)
Location: Multiple Regions Across South Carolina
Job Type: Full-Time / Part-Time / PRN
Position SummaryWe are seeking dedicated and compassionate Nurse Practitioners to join our growing mobile healthcare team serving communities throughout South Carolina. As a provider, you will deliver high-quality, patient-centered primary and preventive care to individuals in residential homes, assisted living facilities, group homes, and other non-traditional settings.
This is a mobile role, and candidates must be comfortable traveling within a 30–45-mile radius of their home location.
Key Responsibilities- Conduct comprehensive physical assessments, diagnostic evaluations, and develop treatment plans.
- Provide acute, chronic, and preventive care to a diverse patient population.
- Collaborate with patients, families, facility staff, and other healthcare providers to ensure continuity of care.
- Perform point-of-care testing and interpret results.
- Maintain accurate and up-to-date patient records using electronic health systems.
- Adhere to all applicable federal, state, and local laws and standards related to patient care and documentation.
- Travel daily to patient homes, assisted living facilities, and similar environments.
- Active Nurse Practitioner license in the State of South Carolina.
- No sanctions or disciplinary actions on current or past licenses.
- National board certification (AANP or ANCC) in family, adult-gerontology, or related specialty.
- Valid DEA license or eligibility to obtain one.
- Current BLS certification.
- Valid driver’s license, reliable transportation, and proof of auto insurance.
- Ability to work independently in non-traditional healthcare settings.
- Strong clinical, communication, and organizational skills.
- 1+ years of clinical experience as a Nurse Practitioner, particularly in home health, geriatrics, or community-based care.
- Familiarity with EMR systems and mobile documentation tools.
Must be able to travel 30–45 miles from your home base to reach patients.
Mileage reimbursement provided.
