* Job Description:

**** Purpose of Job:
** Improve the overall health of the communities we serve by providing counseling to patients on good nutrition habits for their particular situation as follows:###

Essential Duties and Responsibilities:

* Collaborates with providers to coordinate medical and nutritional needs.
* Establishes treatment plans to promote healthy eating and nutritional needs and communicates with patients regarding program compliance or to suggest diet modifications.
* Meets with patients to identify educational needs; provide instruction either individually or in a group setting, utilizes informational handouts/booklets, or refers to community resources as needed.
* Participates in various patient program committees.
* Performs nutritional screenings and offer advice on diet-related concerns such as weight-loss or cholesterol reduction.
* Performs research and recommends ways to expand and improve nutrition programs.
* Participates in quality improvement of the program in conjunction with other services CHAS offers.
* Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.

* Qualifications:

** Education/

Experience:

Bachelor’s degree in Nutrition or related field required.  Registered Dietitian credentialed with the Commission on Dietetic Registration and eligible for state licensure. Experience in chronic disease including HIV and Diabetes and/or maternity nutrition counseling preferred.

Skills:

Excellent oral and written communications skills required.  Computer skills required. BLS (CPR/AED) required. Commitment to supporting a safe and respectful environment is required.
** Physical Demands:
** Dietitian must be able to move around the facility between 1/3rd and 2/3rds of the day, as well as sit at a workstation or in patient rooms.  Using hands occurs over 2/3rds of the day, while reaching with arms occurs less than 1/3rd of the day.  Communicating is also required over 2/3rds of the day, while climbing, stooping or crawling is minimal.
