Key Responsibilities RN Case Manager Responsibilities Complete comprehensive nursing assessments and develop individualized care plans

Provide skilled nursing interventions in patient homes

Manage a balanced caseload with strong organizational efficiency

Coordinate care with interdisciplinary team members

Educate patients and caregivers on disease processes, medications, and safety
