Description:

We are seeking a Hospice Registered Nurse to provide compassionate care in a home health and hospice setting.

The RN will manage a caseload with an average of 15-16 patients.

Ratios are set at 1:1 but may include visits to up to 6 patients per day.

Strong communication skills are necessary for patient and family education and support.

Experience in triage and knowledge of Medicare state regulations are important.

Comfortable dealing with various patient diagnoses and conducting intake assessments is required.

Candidates should be familiar with charting systems and have experience with home health or hospice workflows.

Travelers must have their own transportation to facilitate patient visits.
