Advanced Practice Registered Nurse (APRN) –

Premier Community Health Care is a Federally Qualified Health Center (FQHC) serving Pasco and Hernando Counties since 1979. The APRN delivers comprehensive, patient-centered care, collaborates with an interdisciplinary team, and supports the organization’s mission to create a healthier community for all.
APRN providers will perform general medical procedures, both diagnostic and therapeutic, using minimally invasive techniques to establish, diagnose, and treat acute and chronic conditions, promote health, and prevent disease.
Responsibilities- Identify and treat comprehensive healthcare needs for age-specific populations in collaboration with the supervising physician, within the Nurse Practitioner scope of practice and PCHG policies.
- Provide health education and illness prevention information to promote community health.
- Communicate effectively with all Care Team members to ensure smooth handoffs and a positive patient experience.
- Consult and coordinate with health care providers/clinicians at any location.
- Mentor and train new providers or clinical support team members as needed.
- Collaborate with the Quality Assurance Team and management to develop and implement a quality assurance program.
- Document accurately in Electronic Health Records in a timely fashion in compliance with PCHG protocol.
- Communicate concerns about patient care issues or interpersonal difficulties, following proper chain of command.
- Participate in an after-hours on-call rotation.
- Manage daily and weekly schedules to meet encounter targets and document appropriately in the EHR; review tasks daily to maximize productivity.
- Maintain confidentiality and HIPAA compliance at all times; keep team members informed of disruptions to timely patient flow.
- Contribute to community give-back initiatives and advocate for Premier’s mission.
- Adhere to patient care needs and Premier’s core values to provide a best-in-class patient experience.
- Support department goals and drive Quality Improvement / Quality Assurance (QI/QA).
- Perform other duties as assigned.

- Master's degree from an accredited Nurse Practitioner Program with a clinical specialty in Family Medicine or Pediatrics.
- Current APRN license in Florida.
- Current Drug Enforcement Administration (DEA) license strongly preferred or ability to obtain.
- Current Basic Life Support (BLS) certification.
- Minimum of three (3) years’ experience as a Nurse Practitioner preferred.
- Obtain and maintain Epic certification/annual compliance training throughout employment.
