RN Per Diem Home Health Care Visits with Oasis Documentation - Hernando County
We are seeking Exceptional Registered Nurse (RN) to provide home health care visits with Oasis documentation across Florida. This role involves planning, coordination, and assurance of quality care services to patients and their families.
Responsibilities- Develop work methods and procedures that facilitate the rendering of high quality care.
- Deliver professional nursing services in the client's home setting and provide case management for the primary patients.
- Monitor the changing needs of the client and/or family.
- Provide initial assessment, oversee progress towards goals, monitor clinical outcomes.
- Maintain ongoing communication with patient, families, physicians, other disciplines, Clinical Manager, and office staff.
- Participate in interdisciplinary staff conferences.
- Develop, implement, and continuously monitor the plan of care with the patient and/or representative to ensure quality of care.
- Complete clinical documentation, including OASIS, evaluations, orders, and care coordination records.
- Work with physicians and discharge planners while promoting health and education to patient and/or caregiver.
- Function as a liaison with the community's health‑related resources including DME, oxygen, and infusions.
- Interact with patients, families, team members, and other health care professionals.
- Participate in ongoing education, seeking opportunities for continuous personal and professional development.
- Active Registered Nurse license required.
- Minimum of 1 year clinical experience.
- Home Health/HCHB experience needed.
