Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to deliver in-home health evaluations for members in their own homes, helping bridge gaps in care and improve outcomes. The visit lasts about 45 minutes and includes a history review, physical examination and, if ordered by the health plan, additional diagnostic tests.
You’ll use an iPad workflow; prescribing and lab orders are not required.
