PRN positions are available for Licensed Vocational Nurses (LVN) in the North Houston, TX (Spring, Conroe) area.

We invite you to be a part of one of the fastest-growing Home Health Care Agency and Hospice companies, which is part of a network of healthcare providers spanning six states and employing over 1,800 healthcare professionals.

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
• Document all services provided.
• Demonstrates knowledge of Medicare/Medicaid regulations for home health benefits.
• Report changes in patient condition and needs to RN

Qualifications:

• Current Texas LVN license
• Minimum of 2 years as a Licensed Vocational Nurse (LVN)
• Home Health experience preferred
• Have dependable transportation, driver’s license, and car insurance
