• Registered Nurse will provide, coordinate and direct the provision of home nursing care.
• Perform comprehensive assessments of clients status, including physical, psychosocial and environmental parameters.
• Complete comprehensive assessments OASIS data set within forty-eight (48) hours.
• Participate in a weekend on-call rotation.
• Supervise LVN and CNA's, as applicable.
• Record Plan of Care updates in an EMR - Well Sky experience helpful.
• Follow all The Joint Commission, Federal and State regulations for Home Care and nursing care.
• Develop and follow an individualized Plan of Care that incorporates analysis of data and current scientific findings.

• Current, unencumbered state license to practice as a Registered Nurse in the State of Texas.
• Speak and write English proficiently.
• Professional and engaging customer service skills.
• Typing, writing, and proof reading for accurate, on time clinical documentation.
• Reliable and flexible Clinician.
• Minimum 1-2 years Adult Home Health experience.
• Current CPR certification.
• Ability to assess patient status and identify requirements relative to age specific needs.
• Wound care and Wound VAC experience.
• Reliable transportation, current driver's license, and auto liability insurance.
• Dually vaccinated for Covid-19.
• Flu Vaccinated.
