- Recertification: $80 Tier I, $90 Tier II, $95 Tier III
- Resumption of Care (ROC): $110 Tier I, $120 Tier II, $130 Tier III
- OASIS Follow-Up (SCIC): $70 Tier I, $75 Tier II, $80 Tier III
- OASIS Discharge: $75 Tier I, $80 Tier II, $85 Tier III
- Routine Visit: $45 Tier I, $50 Tier II, $55 Tier III
- SN Evaluation: $55 Tier I, $60 Tier II, $65 Tier III
- SN Discipline Discharge: $55 Tier I, $60 Tier II, $65 Tier III
- Orientation / Training / Meetings: $32 Tier I, $35 Tier II, $39 Tier III
- Provide skilled nursing care in patients' homes
- Complete assessments and individualized plans of care
- Administer medications and treatments as ordered
- Monitor patient progress and changes in condition
- Educate patients and families on medications, disease management, and care needs
- Coordinate care with physicians, therapists, and the interdisciplinary team
- Complete accurate and timely clinical documentation
- Current, unrestricted RN license
- ADN or BSN from an accredited nursing program
- Home health experience
- Knowledge of Medicare and Medicaid guidelines
- Current CPR certification
- Valid driver's license and reliable transportation
- Strong clinical assessment, documentation, and communication skills
