Summary

Under the supervision of the Director of Outreach and Case Coordination, is responsible for all duties related to the development, implementation, and coordination of program plans for the children served within the assigned home(s). Maintains, reviews, and evaluates pertinent program data and information relative to each child's growth and independent development. Ensures that all program plans and behavioral programs are being implemented correctly in the home(s).

Organizes staff training on program related issues such as behavior interventions and active treatment.

What You Will Do
  • Instructs Hope Residential Academy staff in the implementation of the children s daily schedules, including behavior intervention techniques; functions as a leader of the child s interdisciplinary team (I.D.T.); is responsible for recognizing the need to call IDT meetings as well as conducting the meetings; assembles all relevant information for IDT meetings; prepares Implementation Strategy/ISP, and ensures inclusion into the child s agency record.

    These I.D.T. meetings can include but are not limited to pre-admission, admission, discharge, education special staffing, thirty day evaluation, and interagency staffing s.
  • Participates in all meetings or decisions concerning children or services provided in the Plex assigned. Participates in regular meetings with the Residential Academy staff and other Hope staff to discuss and address programmatic issues concerning the children.
  • Coordinates, develops, implements, and maintains individual program plans for each child within the Plex assignment. This includes the child s permanent record, all quarterly and annual reports (narratives, graphs, etc.), and parent/guardian staffing s.
  • Communicates regularly with parents, guardians, and other resource persons/consultants regarding the child s care, issues, and program implementation and acts as liaison with community service providers and agencies.
  • Reviews each child s progress at least monthly, and revises or replaces programs as necessary.
  • Keeps all essential paperwork up to date which includes but is not limited to the ISP, ICAP, Assessments, Monthly progress notes, Plex meeting notes, and Monthly calendars.
  • Attends and prepares reports for DCFS which includes the youth s Annual Case Review (ACR) along with monthly and/or quarterly meetings.
  • Make monthly rounds to assigned homes to check on youth s progress, behaviors, etc.
  • Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
  • Serves as a member of daily rounds, residential review, referral review, best placement, attends screenings, behavior intervention committee, behavior treatment committee, feeding team, behavior clinic and other pertinent meetings.
  • Make requisitions for youth s clothing and/or personal needs.
  • Help families obtain guardianship of youth as they turn 18.
  • Inservice all new DSP mentees in assigned plex.
  • Facilitate transitions to adult placements or other outside facilities, and between Hope homes.
  • Performs other duties as required or assigned.
What You Will Bring
  • Bachelor’s degree in a Human Services Field
  • At least one year of experience working directly with people with intellectual disabilities or other developmental disabilities
  • The ability to drive and qualify as a DCFS approved driver
  • Requires ongoing certification and administration of Safety Care and CPR practices and techniques.

  • Master’s degree in a Human Services related field
  • A minimum of 3-5 years’ experience as a QIDP or support role with children’s group homes
