Position: Medical Social Associate (4260)
Description
This position provides direct services to individuals. This program is funded to provide trauma informed care for patients/clients who do not often fit well within the standard medical model. This position will work closely with Survivor Recovery Center (SRC) staff, medical providers, primary care teams, and other agencies to improve patient outcomes and care. This position will conduct outreach and assertive case management to survivors of violent crime who are typically unable to access traditional services.
This position will work closely with primary care providers, Local Health Departments, and Community Based Organizations to provide wrap-around services and create a foundation of trust for continued care of socially vulnerable populations.
Examples of Duties
Determines client eligibility for services.Interviews clients and/or families regarding needs related to social determinants of health, which may include home visits and coordinates the specific social and/or medical services recommended.Assists patients in setting and achieving personal goals in person, by phone or in group setting.Through skill building interventions and other supportive interventions, assist patients in effectively utilizing a wide spectrum of resources, managing symptoms of their illness, and developing daily living skills to gain greater independence, employment and other meaningful activities.Initiates or recommends referral services to other agencies in meeting patient needs.Develops and coordinates service plans which may include collaboration among agencies to recruit and organize a network of integrated services.Maintains current case records and prepares reports as required.Collects and maintains current information about available community resources.Arranges and participates in interdisciplinary case conferences as needed.Explains university/agency services and programs to the community, and/or delivers educational presentations and materials to community organizations.Communicates unmet client needs to responsible parties for appropriate action.Participates in interagency collaboration initiatives.Acts as liaison between patients and providers to facilitate communication.Advocates for clients' rights.Performs other related duties as assigned.Qualifications
One of the following:- a) Bachelor's degree with a major in the social or behavioral science or related health specialty from an accredited college or university;
OR - b) Master's degree in social or behavioral science or related health specialty area from an accredited college or university;
AND
One (1) year (12 months) of progressively more responsible full-time experience in the practice of social services/agencies or medical/clinical setting maintaining acceptable standards of social work practices.Supplemental Information
