Overview

Northeast Center for Youth and Families, Inc. has an opening for two full-time Clinical Case Manager/Educational Liaisons (CCM) in Residential Services who will focus on clinical, collateral case management, ongoing assessments, and aftercare. The CCM will also act as the Educational Liaison to support the youths' learning and to ensure their IEPs are comprehensive and meeting their needs in the school environment.

This CCM will act as an advocate for the youth in the educational setting. Availability during school hours, attending school meetings, and being available during after school hours to establish relationships with students and provide for needs that emerge is required as well as after hours on-call support to the programs.

Clinical case management, collateral case management, ongoing assessments, and aftercare; act as Educational Liaison to support youths  learning and ensure IEPs are comprehensive and meeting needs in the school environment; advocate for youths in educational settings; be available during school hours, attend school meetings, and be available during after school hours to address needs, with after-hours on-call support to the programs.

• A Master s Degree is required.
• Candidates must have a valid driver s license with approved DMV, DCF, and CORI background checks. Travel is involved and a reliable vehicle is needed.
