Position Purpose
The Community Health Worker assists individuals in the local community gain access to key social service and healthcare providers/services to better their health and improve their lives. This position is based out of Mercy Medical Center.
What You Will Do
- Improve access to health and human services for target population through in-person interactions, telephone contacts, and coordination of referrals
- Provide transportation as necessary
- Facilitate enrollment in available federal, state and local programs to increase access to health care and support services– examples include social security benefits, food, and housing services
- Utilize a variety of teaching strategies, including verbal, handouts, and/or video based on participant learning style
- Provide basic education regarding chronic health conditions
Minimum Qualifications
- High School diploma or equivalent
- Current Basic Life Support (BLS) certification
- Must possess a comprehensive knowledge of the local community
- A current driver's license in good standing and the ability to drive a vehicle or have access to reliable transportation is required
- Requires the ability to communicate with target population
Position Highlights And Benefits

Pay Range $21/hr – Pay is based on grant-funding.

Mercy Medical Center is a 182-bed acute care hospital in Springfield. Other facilities include Mercy's Rehabilitation Hospital and Brightside for Families and Children.
