We’re in search of a case manager to work on a full time basis for our Recovery Now program, part of our Behavioral Health & Recovery Services Division. The case manager is accountable to the Program Director, who is responsible for providing supervision and training. The case manager is one who actively promotes the philosophy of the Gándara Center and maintains a high level of commitment to the program.
Dutiesand Responsibilities
- Provide assessment and service planning to address OUD and identified social determinants of health
- Develop, implement, and monitor service plan consistent with identified needs
- Communicate effectively and in a timely manner with persons served and collaterals
- Attend external and internal meetings relevant to individual served and/or services
- Ensure that persons served progress notes, service plans, and other required documentation is completed professionally and accurately and in the electronic health record in a timely manner
- Ensure that consultation on the person’s served behalf is conducted
- Provide wraparound care and serve as primary point of contact for individual served
- Attend relevant trainings as required by Program Director
- Bachelor’s Degree in psychology or related field
- Specialized training/experience in child/adolescent development, behavioral treatment, substance use/dual recovery issues, and family treatment/engagement/education regarding psychiatric and substance abuse recovery issues
- Must understand the population and information sensitivity (i.e. HIPAA and privacy laws and regulation) protocols required in mental health
