CSO is looking for an Integrated Care Nurse for our clinic in Springfield. This Nurse will provide medical care and treatment for clients, ensuring coordination of care and appropriate discharge planning. We encourage newly licensed nurses who have an interest in behavioral health to apply. This entry level position provides the opportunity to practice as a nurse under the leadership of a highly compassionate, understanding, and skilled nursing leadership team.

This position is a Monday-Friday 1st shift position—no nights, no weekends, no major holidays, and no on-call requirement!

You Will

• Provide medical care and monitoring for program clients, including medication order fulfillment and administration
• Contribute to the Comprehensive Assessment and care planning of engaged program clients, including evaluating for substance use and withdrawal
• Assist in the evaluation of clients in need of emergency treatment services for risk management, safety planning, and possible hospitalization, providing crisis counseling and crisis prevention planning as needed
• Contribute to the development, adaptation and execution of transition plans, including medication reconciliation process as appropriate
• Provide in-service training for staff on common medical issues and appropriate treatment, intervention and support strategies as well as provider psycho-education
• Conduct medication reviews and monitoring as needed
• Complete necessary reports and paperwork in a timely manner
• Attend in-service trainings on evidence-based best practices and other topics as appropriate

Your Background

• Must be licensed in Massachusetts as a Registered Nurse
• Valid driver's license and reliable transportation
