Floating Support Specialist – Holyoke and Springfield Young Parenting Program

The YWCA of Western MA is dedicated to eliminating racism and empowering survivors. The YWCA provides a safe place for survivors of domestic violence, sexual assault, and human trafficking. Our programs and services include crisis shelter and transitional housing for survivors and their families, counseling, SAFEPLAN court advocacy, community education, supervised visitation, and vocational training for youth. The YWCA provides services at several sites, including Holyoke, Northampton, Westfield, and our primary Springfield campus.

Position

Purpose

Reporting to the Program Director, the Case Manager is responsible for the case management responsibilities for the Young Parenting Program. They will empower residents with life skills, employment search, housing, budgeting, and advocacy and community resources to assist them with transitioning into safe appropriate housing. The Case Manager will assist residents with employment and housing opportunities in the community. This role follows the agency mission and trauma‑informed service model to provide the support and advocacy needed for this population.

Responsibilities
  • Pre‑intake, intake, assessment, treatment planning and case management for all residents, including Bio‑Psycho‑Social Assessment, CANS (DCF families), treatment plan, transition plan and child assessment to include developmental screening by EI provider within 45 days of intake.
  • Collaborate and engage with family, peer advocates, family partners and other involved partners in care of participant to ensure a positive transition into the program.
  • Engage services and activities with input from participant and other members of the participant’s team to identify mutually agreeable and measurable goals and objectives and to promote appropriate and integrated services.
  • Develop treatment plan within two weeks of intake and rewrite plan every three months, holding a team meeting for every review and including all involved family and service providers.
  • Refer participants for individual therapy, seek referrals for medication evaluation, and consult clinical team members as appropriate.
  • Develop behavior support plans with participant, family, and staff.
  • Provide crisis counseling as needed.
  • Provide direct assistance to clients by helping them to obtain needed services to enable them to transition to community, including housing search and advocacy.
  • Monitor and document client progress.
  • Maintain complete and timely participant records.
Qualifications
  • Associates Degree with 3 years of direct experience in human services field.
  • Bachelor’s with 1 year direct experience in human services field preferred.
  • Residential experience and experience with adolescents and/or young parents and children preferred.
  • Bi‑lingual (Spanish/English) skill preferred.
  • Experience with survivors of gender‑based violence and exploitation.
  • Experience addressing the impacts of decisions and actions on multiple communities.
  • Understanding of both interpersonal and structural racism and bias.
  • Ability to represent agency from a management perspective in an active union environment.
  • Excellent written and organizational skills including computer skills.
  • Must have knowledge of/contacts with community agencies.
  • Reliable transportation, safe driving record, and valid driver’s license required.
  • Must be able to be covered by the agency’s insurance policy.
  • Must be able to lift 40 lbs.
