Position SummaryThe Family Engagement Coordinator provides outreach, assessment, case management, and intervention services to youth ages 16–24. Working with youth, families, schools, and community partners, the Coordinator connects participants to resources and opportunities that promote safety, positive development, and long-term success.

Youth Outreach & Engagement- Provide community-based prevention education, outreach, intervention, and case management services to youth ages 16–24.
- Welcome, screen, and engage youth seeking program services.
- Build positive relationships with youth and support ongoing participation in services.
- Conduct assessments to identify participant needs, strengths, barriers, and appropriate services.
- Provide ongoing case management and support participants in establishing and working toward individual goals.
- Maintain appropriate communication with youth and families to support participant engagement and progress.
- Connect youth with community resources, including substance use services, counseling, tutoring, mentoring, vocational training, employment resources, and other needed supports.
- Provide referrals and linkages based on identified participant needs.
- Assist youth in navigating community-based services and reducing barriers to accessing resources.
- Support positive behavioral, educational, employment, and personal development outcomes.
- Support community-based strategies designed to prevent and reduce youth violence.
- Foster relationships among youth, families, schools, healthcare organizations, social service agencies, workforce providers, businesses, and public agencies.
- Participate in collaborative meetings, committees, and community initiatives that support youth and young adults.
- Travel to schools, community organizations, businesses, and other program sites to provide services and maintain partnerships.
- Maintain accurate and timely participant records, daily service logs, case notes, referrals, and program reports.
- Collect and maintain information required to monitor program participation and outcomes.
- Assist with data collection and review to support program evaluation and adjustments in accordance with funding requirements.
- Maintain confidentiality and comply with NNCC policies and applicable program requirements.
- Participate in required trainings and professional development, including Trauma-Informed Care, Youth Development, Motivational Interviewing, and related topics.
- Maintain flexibility in scheduling based on participant and program needs.
- Perform other related duties and projects as assigned.
- High school diploma, GED/HiSET, or equivalent.
- Minimum of one year of experience working with adolescents, young adults, or community-based programs.
- Experience or training in youth development, violence prevention, community outreach, human services, case management, or a related area.
- Ability to engage youth and work effectively with individuals from diverse backgrounds.
- Strong communication, interpersonal, organizational, and documentation skills.
- Ability to collaborate effectively with schools, community organizations, healthcare providers, social service agencies, and other partners.
- Basic proficiency with computers and applications such as Microsoft Office, email, and internet-based systems.
